SLFRF Grant Agreements
Grant Agreement and Intergovernmental Grant Agreement templates
The Office of the State Controller has completed templates for a new Grant Agreement and Intergovernmental Grant Agreement for use with State and Local Fiscal Recovery Funds (SLFRF). The templates include provisions that the State agreed to prior to the U.S. Department of Treasury sending SLFRF funds to the State. Every subrecipient using SLFRF funds must sign both the Grant or Intergovernmental Agreement and the Certification Agreement to acknowledge and comply with the Treasury’s requirements. These agreements are mandatory.
The agreement package must be used for all SLFRF subrecipients and includes the following:
- Standard grant agreement language and provisions
- Exhibit A Statement of Work - to be completed by the agency
- Exhibit B Sample Option Letter
- Exhibit C Budget (including SLFRF Budget Modification Form)
- Exhibit D Federal Provisions (including SLFRF reporting and compliance requirements)
- Exhibit E Subrecipient Certification Agreement
- Exhibit F SLFRF Subrecipient Quarterly Report requirements (including SLFRF Subrecipient Quarterly Report workbook)
- Exhibit G Sample SLFRF Reporting Modification Form
The OSC will be providing training on this agreement package during SLFRF Office Hours on December 16, 2021 at 3 pm.

After an agency receives a completed Grant Agreement or Intergovernmental Grant Agreement and the Certification Agreement from a subrecipient, the agency will save these agreements in the subrecipient’s grant file. Contact stacey.alles@state.co.us if you have questions.
