Dear Primary Care Provider,
As a trusted source of information for their patients, primary care providers play an important role in the effort to increase COVID-19 vaccination uptake in our state. The Colorado Department of Public Health and Environment (CDPHE) is inviting your primary care practice to enroll in the COVID-19 Vaccination Program as part of its efforts to quickly mobilize additional COVID-19 vaccine providers across the state and to increase access to COVID-19 vaccination for all Coloradans. As we transition from a mass vaccination model to more provider-focused efforts, we need your help now more than ever.
To prepare your practice for participation in the COVID vaccine program and to answer your questions, CDPHE is hosting a town hall for primary care providers in collaboration with Governor Jared Polis tonight at 6 p.m. The town hall will include remarks from Governor Polis and a deep-dive into COVID-19 vaccine program requirements and important resources followed by a 30 minute Q&A session.
In the interim, we’re also sharing our Primary Care Provider Communications Toolkit which includes ready-to-use email, text, and social media messages you can use to encourage your patients to get vaccinated or to set up an appointment once your clinic becomes a COVID-19 vaccine provider. We hope you can join us on May 18 to learn more!
Program Requirements
To receive publicly-funded COVID-19 vaccines, vaccine diluent, and ancillary supplies at no cost, your facility must agree to adhere to all of the requirements specified in the CDC COVID-19 Vaccination Program Provider Agreement, adhere to all the CDC Storage and handling requirements and complete the CDC’s Storage and Handling Training.
In addition, to be approved for enrollment as a new COVID-19 vaccine provider, a virtual enrollment site visit must be completed and the following documentation must be submitted to CDPHE for review:
- 5 days of stable temperatures documented via a digital data logger thermometer.
- 5 days of paper temperature logs where the current temperature is documented twice a day and the minimum and maximum temperature recorded is documented once a day.
- A completed Emergency Vaccine Management Plan.
- Pictures of the glycol bottle placement inside the storage unit, which is attached to the digital data logger thermometer.
- Pictures of storage unit outlets with “DO NOT UNPLUG” tags in place.
Provider enrollment for the CDC COVID-19 Vaccination Program occurs online through CDPHE’s Pandemic Vaccine Provider Registration portal. If you have more than one facility identified as a provider, you will need to enroll each location separately.
Please complete your facility’s enrollment by following these steps:
- Open the Pandemic Vaccine Provider Registration Portal link: COVID-19 Enrollment Link.
- Click the red “Click to Register” button near the middle of the screen.
- Complete the CIIS enrollment and sign the Letter of Agreement (LOA).
- Click on Enroll as Pandemic Provider to request COVID-19 Vaccine. Answer all required questions to complete your facility’s COVID-19 enrollment. COVID-19 Pandemic Enrollment Required Information List
