Required EDI Reports for Legacy Claims
If a Legacy Claim was not properly filed, meaning a FROI R1 MTC (First Report of Injury submission using the “R1” Maintenance Type Code) was required but not submitted to the DOWC, a Legacy Claim would not exist to match with a FROI UR (First Report of Injury unreported). In this case, the DOWC requires submission of the appropriate FROI 00 (Original), 04 (Denial), or AU (Acquired/Unallocated) report instead of a FROI UR.
After submitting a FROI 00, 04, or AU, an SROI UR (Subsequent Report of Injury unreported) may be filed if SROI events have happened and have not yet been reported to the DOWC. This applies only if the date the claim administrator became aware of the injury (DN0041) is before July 30, 2026. If the SROI UR is not submitted, the DOWC will accept the appropriate SROI report per the Event Table and sequencing guidelines.
Please review the Event Table for specific reporting requirements. The DOWC requires all FROI and SROI UR report submissions to be completed by October 31, 2026. If a claim reopens after October 31, 2026, the same reporting requirements apply.
