- Headcount is not carried over from the prior year.
- Headcount includes all payroll employees nationwide who worked 20 or more calendar workweeks in 2025, regardless of full-time or part-time status.

Private Plan Self-Insured Employers and Private Plan Carriers are required to report aggregate claim information.
The 2025 Q4 reporting deadline was January 31, 2026. Amended reports must be submitted no later than March 1, 2026.

2023 Premium Overpayment Refund Checks – Certification Required
Deadline – As Soon As Possible
Paper refund checks for outstanding 2023 FAMLI premium overpayments were mailed to qualified employers at the most recent address associated with their FAMLI account. Each check included a certification form that must be signed and returned to the FAMLI Division.
The certification confirms your acknowledgment of the responsibility to refund any employee contributions that were previously withheld. Please sign and return the certification as soon as possible to complete the refund process.
- Employers who do not return the signed certification within six months may be referred to the FAMLI Audits team for review.
- Uncashed refund checks will escheat after six months and be transferred to the Treasury as unclaimed property.
If you have questions or need another copy of the certification, please contact 866-CO-FAMLI (866-263-2654), Monday through Friday, 7:30 a.m. to 4:30 p.m.
