You asked, we listened: Updates to the FAMLI Serious Health Condition Form
Health Care Providers have given us invaluable feedback to improve our Serious Health Condition form. We’re making updates to the pregnancy and childbirth section to make it more clear whether the claim is related to pregnancy or childbirth and whether a complication was involved.
For intermittent leave claims, providers will also soon be able to enter the number of hours a patient may be unable to work per a 30-day period. This more closely aligns with what you’re used to seeing on FMLA paperwork.
Expect to see those improvements by the end of September!
Don’t forget that incomplete fields on these forms can delay payments for your patients. So whenever you complete a Serious Health Condition form, make sure any field listed as “required” is filled out!
If you omit any required fields, your patient will have to contact you again to have it corrected. So help us help your patients by calling for personalized assistance anytime you see a required field that you’re not sure how to answer. Tell Your Patients about FAMLI
After-visit summaries are super important for health care providers. They let patients know their next steps and give them crucial information about their health.
Now you can make those summaries even more valuable by letting your patients know about paid family and medical leave in Colorado. We encourage you to copy-and-paste the message below into your after-visit summaries:
Did you know that nearly all workers in Colorado now qualify for paid leave to take care of themselves or a loved one with a serious health condition? Pregnancy qualifies as a serious health condition, and both parents are eligible to take up to 12 weeks to bond with a new child. Find out more and get your claim started at famli.colorado.gov.
We also encourage you to print out our What-Is-FAMLI flyers in both English and Spanish. These flyers are a great introduction to FAMLI and will point your patients in the right direction to learn more.

It’s not too late to cut the paperwork!
We understand that health care professionals do not go into medicine to do paperwork.
We created the Health Care Provider portal in My FAMLI+ to help make things easier for you and your patients when they or their loved ones need to step away from work to take care of a serious health condition. Health care providers are vital in making sure that the FAMLI program is an accessible and meaningful benefit for workers.
Register and create an account in My FAMLI+ today. After you register you must upload a copy of your medical license. FAMLI must verify your medical license before you are able to certify your patients’ serious health condition forms.

Add Your Delegated Staff
Having a delegated staff member makes certifying your patients’ claims easy!
Remember, this is your account, and you are the primary account holder. Only your license should be uploaded to the account for verification.
But a delegated staff member can help. Adding a delegated staff member to your account can elevate the patient experience by speeding the process.
The result? Faster claim-processing times for your patients, so they can focus on their prescribed treatment plan, maintain their health insurance coverage, and receive partial wage replacement when they’re unable to work.
