At FAMLI, we understand that health care professionals do not go into medicine to do paperwork. We created the Health Care Provider portal in My FAMLI+ to help make things easier for you and your patients when they or their loved ones need to step away from work to take care of a serious health condition. Health care providers are vital in making sure that the FAMLI program is an accessible and meaningful benefit for workers.
Register and create an account in My FAMLI+ today. After you register you must upload a copy of your medical license. FAMLI must verify your medical license before you are able to certify your patients’ serious health condition forms.

Serious Health Condition Form
When you complete a Serious Health Condition Form for your patients,it is important to provide all of the required information. We have indicated which fields on the form are required with an asterisk. If you omit any of the required fields, it will result in your patient having to contact you again to have it corrected.
FAMLI has heard feedback from both patients and health care providers, and we’re in the process of making some updates to the Serious Health Condition Form. The changes are intended to make it easier to manage pregnancy and childbirth claims. Stay tuned, more information will be coming soon!

Add Your Delegated Staff
Having a delegated staff member makes certifying your patients’ claims easy! Remember, this is your account, and you are the primary account holder. Only your license should be uploaded to the account for verification. If you're registered and your license has been verified in My FAMLI+, you or your delegated staff member will provide an elevated patient experience by eliminating paperwork. This can result in faster claim-processing times for your patients, allowing them to focus on their prescribed treatment plan, maintain their health insurance coverage, and receive partial wage replacement when they’re unable to work.
