Serious Health Care Conditions Forms
Important reminder! When you complete a Serious Health Condition Form for your patients, please be sure to provide all of the required information. We have indicated which fields on the form are required with an asterisk. If you omit any of the fields that are required, it will result in your patient having to contact you again to have it corrected.

Intermittent Medical Leave Claims
Intermittent medical leave claims must be certified by a health care provider using weekly parameters. If you are not able to certify your patient’s need for leave in weekly parameters, please encourage your patients to file their intermittent leave claims retroactively each time they’ve missed work.

My FAMLI+ allows you to add an administrator or supporting medical staff to your account as a delegate. Before you do that, you will need to set up your own account. After your account is set up, you can add delegates. Remember, this is your account and you are the primary account holder, only your license to practice should be uploaded to the account for verification. If your chosen delegate is also a health care provider, they must not upload their license to your account. They are certifying medical claims on your behalf.
