CMHHIP Team,
As a reminder with flu season creeping up: anyone on the CMHHIP campus who tests positive for COVID-19 must report the CDPHE required information to the Infection Control department within 24 hours. The supervisor is accountable to ensure this is done.
There are two ways to report:
1. Input all of the required information into this form. All questions must be answers (have your COVID-19 vaccination dates handy)
