Dear Valued Provider,
Thank you for engaging with the Colorado Universal Preschool Program. On a yearly basis, the Colorado Department of Early Childhood (CDEC) must enter into new agreements with providers that clarify the responsibilities of CDEC and those of providers as they work together to enroll children into the Colorado Universal Preschool Program.
You are receiving this notice today as you have been identified as the designated Authorized Representative for your organization through the Preschool Program Provider Portal. The Authorized Representative may be a CFO, Superintendent, business official or other leader authorized to sign the Provider Agreement. If you are not the Authorized Representative, please respond to this email and identify the individual who should be the organization's Authorized Representative and include that individual as a party to the email response.

Please forward this boilerplate agreement to all necessary individuals who need to review and approve the document within your organization. This will allow any needed revisions to the agreement to be incorporated into the finalized version for execution. Additionally, reviewing this document now will allow for a more timely execution of the final agreement. The Department requires a response from your organization no later than March 15th, 2024 by 5 pm in order to ensure timely execution of agreements as matching occurs in April.
Items CDEC requires from your organization to complete execution:
Current Insurance Coverage - After your organization has reviewed and approved the agreement, please include a copy of your current insurance coverage. This is a required document from the state to ensure providers have adequate insurance coverage. This can be found in section 10 of the Provider Agreement. A sample insurance example can be found HERE for clarification of what is required.
Registered and in good standing with the Colorado Secretary of State - Please ensure your organization is in good standing and current in its registration with the Colorado Secretary of State. Please confirm in your response to CDEC that your organization is in good standing. The state can only work with organizations that are in good standing with the Colorado Secretary of State. CDEC will also confirm your good standing with the Colorado Secretary of State before issuing the vendor agreement to your organization for execution. Vendors can choose to either submit a copy of their Colorado Secretary of State registration in good standing as a PDF attachment, or vendors can provide their Colorado Secretary of State registration number for our verification.

Finalizing the Agreement
The below items will need to be confirmed and provided via email to CDEC
CDEC_procurement-contracts@state.co.us. Once CDEC has received your confirmation and acceptance of:
- An affirmative acknowledgement that you accept the vendor agreement
- a copy of your current insurance coverage, and
- A copy of your Colorado Secretary of State registration showing your organization is in good standing in PDF, or, your registration number with the Colorado Secretary of State so CDEC can confirm this.
The Department will email the Provider Agreement via DocuSign for signature from the Authorized Representative. The vendor will be responsible for entering the legal name of the provider, address of the provider, and signatory information (name, title) when executing the document in DocuSign. CDEC will enter the license numbers associated with the provider.
If you are part of an entity that has multiple locations, only one Provider Agreement will be sent to the Authorized Representative, and that will include all locations participating in the Colorado Universal Preschool Program.
After the Provider Agreement is signed and submitted by the Authorized Representative, the authorized CDEC delegate will also sign the Provider Agreement. With both signatures, the Provider is approved to participate and enabled for payment.
