CONSENT ITEMS

1.   Housing and Human Services:  Contracts and Task Orders
      a.  Agreement with Boulder Valley School District to
           fund Eligibility Technicians to determine eligibility
           for Medical Assistance, CHP+ and Food Assistance;
           conduct education & outreach, and link clients to
           Connect for Health Colorado ($51,200 revenue);
      b.  Contract renewal No. 1 with Empowering Choices Therapy,
           LLC for therapeutic services (not to exceed $50,000),
           with amendment No. 1 to include updated scope of
           services, updated fee schedule, response to SOQ 6261-15
           and indemnity;
      c.  Contract renewal No. 1 with Excelsior Youth Center for
           therapeutic services (not to exceed $100,000), with
           amendment No. 1 to include updated scope of services,
           updated fee schedule, response to SOQ 6290-15 and
           indemnity;
      d.  Contract renewal No. 1 with Specialized Alternatives
           for Families & Youth of America, Inc. for therapeutic
           services (not to exceed $580,000), with amendment No. 3
           to include updated scope of services, updated fee
           schedule, supported work model scope of services,
           supported work time sheet and progress report; response 
           to SOQ 6290-15 and indemnity;
      e.  Contract renewal No. 2 with TLC Learning Center for
           general operating support (not to exceed $25,000),
           with amendment No. 2 to include scope of services, fee
           schedule and HIPAA language;
      f.   Contract renewal No. 2 with The Wild Plum Center for
           general operating support (not to exceed $31,518), with
           amendment No. 2 to include scope of services; payment
           and reporting, and HIPAA language; 
      g.  Contract renewal No. 2 with Youth Advocate Programs for
           therapeutic services (not to exceed $150,000), with
           amendment No. 3 to include updated scope of services,
           updated fee schedule, supported work model scope of
           services, supported work time sheet and progress report;
           response to SOQ 6261-15 and indemnity;
      h.  Task Order 2017-01A to Master Contract with the YWCA of
           Boulder County for general operating support (not to
           exceed $79,825).

2.    Area Agency on Aging:  Amendment No. 4 to the chores and
       transportation services contract with Boulder County
       CareConnect, to increase the not-to-exceed amount from
       $110,086. to $115,086.

3.   Commissioners’ Office:  Worthy Cause contract amendment No. 2
      with the Immigrant Legal Center of Boulder County to transfer
      2011 funding from unit #6 to unit #9 of the same property,
      located at 948 North Street, Boulder ($150,000 WC III funding).

4.   Commissioner’s Office:  2017 Economic Development Contracts
      a.  Niwot Business Association ($2,325);
      b.  Town of Lyons ($6,000).

5.   Boulder County Head Start:  Continuing grant award from the
      Temple Hoyne Buell Foundation for an Early Childhood Mental
      Health Consultant ($60,000).
      ACTION REQUESTED:  Approval / Signature

6.   Parks and Open Space Department:  IRS Noncash Charitable
      Contributions Form 8283 for the Les & Martha Williams Conser-
      vation Easement property.
      ACTION REQUESTED:  Approval / Signature

7.   Parks and Open Space Department:  Grant-funded contracts for
      Wildfire Defensible Space Treatment
      a.  Wildfire Protection Pros, LLC (south vegetation removal
           $94,350); 
      b.  RMRP Enterprises dba Rocky Mountain Resource Protection,
           LLC (central vegetation removal $125,000).
      ACTION REQUESTED:  Approval / Signature

8.   Transportation Department:  Contract with Flatirons Transpor-
      tation dba Green Ride Boulder for the Hessie Trailhead Shuttle
      Service ($99,381).

Elected Officials and Department Heads Matters

9.   Community Services Department:  Extension and Amendment of
     Lease Agreement with The Wilderness Early Childhood Center,
     LLC, for Boulder County’s Head Start Program located at 2845
     Wilderness Place, Suite 102, Boulder.  The current lease rate
     is approxi-mately $37,000 and expires on 7/20/17.  The extension
     term is for one year (7/20/17 to 7/20/18) for a negotiated fixed
     lease at a rate of $52,819.23.
     ACTION REQUESTED:  Approval / Signature 

10.  Land Use Department:  Docket LU-17-0003 / SPR-17-0015 Treves /
       Grosz Residence and Driveway; Final design based on the May 11,
       2017 public hearing for 1,183 cu.yds.of earthwork for construction
       of a driveway and site improvements, and construction of a 4,126
       sq.ft. residence, a 93 sq.ft. gazebo, and a 136 sq.ft. trellis
       structure.
       ACTION REQUESTED:  Decision

11.  Budget Office: 2017 Budget Adjustment Request from the Transportation
       Department ($107,004).

12.  Commissioners’ Office:  Boards and Commissions
       a.  Lyons Regional Library District Board of Trustees -- Drew Biel has
            resigned  and Erin Biesiada was appointed to serve his existing
            term, which ends on September 1, 2018;
       b.  Ben Blaugrund has resigned from the Planning Commission;
       c.  Kristie Sykes has resigned from the Cultural Responsiveness and
            Inclusion Advisory Committee.
