We invite you to share the Community Action Plan (CAP) Community Needs Assessment Survey with San Diego community members.
The survey is open to all residents and provides an opportunity for community members to share their perspectives on local needs and priorities. The survey results will help inform our Community Health Assessment (CHA) and future community planning efforts for the Community Health Improvement Plan (CHIP).
The survey will remain open through Friday, July 31, 2026.
Please see the links below for flyers that include the survey link and a QR code. There is a link for each of the County’s threshold languages. We encourage you to share these materials through your networks and other community outreach efforts.
