Dear BHS Providers,

The BHS Data Science team has developed a standardized SmartCare Report Request Form to streamline requests related to reports within SmartCare.

This form should be used to:

Request a new report
Request enhancements to an existing report
Request corrections or fixes to a current report

The SmartCare Report Request Form is attached to this email for your convenience. It is also available on the Optum website for ongoing access (SMH & DMC-ODS Health Plans page > SmartCare tab > Data and Reporting section).
