Medi-Cal Subscriber,
You have been mailed a renewal form because your county needs information from you to make sure you keep your Medi-Cal coverage. Medi-Cal covers doctor visits, In-Home Supportive Services (IHSS), dental care, hospital stays, prescription drugs, mental health services, and more for you and your family.
You must return the Medi-Cal renewal form or the information the county asks for to see if you can keep your Medi-Cal coverage. If you do not submit the renewal form or the information the county asks for by the due date listed on the form, you may lose your Medi-Cal coverage.
Look for a large yellow envelope coming to your mailbox soon. The easiest way to complete your form is online. You can also give it to your local county office by mail, in person, or over the phone.
If your county determines you’re no longer eligible for Medi-Cal, you may qualify for financial help to pay for a private health plan through Covered California. Your county will share your information with Covered California to make sure you don’t lose health coverage.
