Make The Most of Your Visits
Well Child Visits are yearly check‑ups for everyone from birth through age 20. They help kids, teens, and young adults stay healthy, keep up with vaccines, and get support as they grow. When it's time, these visits also help families get ready for the move from pediatric to adult care.
Here are a few easy things you can do to prepare for your visit:
Ages 3-13
At this age, getting ready for your child’s visit helps the doctor understand how they are growing and what support they may need.
📝Write down your child's changes in growth, behavior, sleep, or school.
💊Bring a list of their medicines or vitamins.
❓Think of any questions you want to ask.
😊Remind your child the visit helps keep them healthy.

Ages 14–16
As your teen becomes more independent, getting ready for their visit helps them practice taking care of their own health.
✍️Have your teen write down their own questions.
🙋♂️Let them practice checking in and speaking for themselves.
🏫Bring needed school or sports forms.
🔍Talk about habits like sleep, stress, eating, and screen time.

Ages 17–20
As your child becomes a young adult, their health care will begin to change. If their doctor is in Family Medicine, they may be able to stay with that doctor. If their doctor is a pediatrician, they will move to an adult doctor.
Before they turn 21
🏥They can still go to their pediatric clinic. You can help them track their appointments and make sure they go.
💊Their doctor will check their medicines, refills, and referrals. They will talk to them about next steps. This is a good time to check together what care they’ll need.
📖Help them learn their health history and ask questions about moving to adult care.
After they turn 21
🔁 Their insurance will automatically assign them an adult care doctor within LA Health Services. You can help keep an eye out for messages about the new doctor.
📞 Their new clinic may call to schedule a first visit. Support them in answering the call or help them make the appointment.
❓ If you’re unsure who their new doctor is, you or your child can call the current clinic or their insurance plan to confirm. Teens and parents can make this call together.
