HEALTH BENEFITS - OPEN ENROLLMENT 2023
If you are currently enrolled in a CalPERS Health Plan you will soon receive a package in the mail from CalPERS containing the 2023 CalPERS Health Program Guide and the 2023 Health Benefits Summary Booklet. If you do not receive a packet, you can request the information by visiting the CalPERS website or you may contact them at their toll-free number 1-888-225-7377.
IMPORTANT DATES:
Open Enrollment Period: September 19, 2022 – October 14, 2022 (Please do not turn in Open Enrollment forms prior to 9/19/22)
Effective Date of Change: January 1, 2023
WHAT DO YOU NEED TO DO?
If you do not wish to make any changes to your health coverage, you do not need to do anything.
If you do wish to enroll in or make a change to your plan, please follow these steps:
- Review the Benefit Plan Rate Sheets below that pertain to you as a Glenn County employee (DSA, GCMMA, SAA, General Unit, POA or Non-Represented) along with your 2023 Health Program Guide and Benefits Summary Booklet. Then decide what plan is right for you.
- Complete the Health Benefits Enrollment Form – CalPERS Health Enrollment.
You will need to complete boxes: 1 through 7, 9 through 13 (the Permitting Event date will be the date you fill out the form, within the Open Enrollment period only), 14, 15 (Please fill out for yourself and add all applicable dependents (See Relationship Codes at bottom of box. Remember to check the Action box that pertains to your enrollment), 16 OR 17, sign and date boxes 18 and 19. The Personnel Department will fill out Section H on the back of the form.
NOTE: In Box 15 – you must list ALL dependents you wish to have enrolled in the plan. If you do not list a dependent that you currently have covered, we will assume that you have dropped that dependent.
- If adding NEW dependents, you will need to provide a Birth Certificate or Marriage License (as applicable). You must also provide the dependent’s social security number.
- Submit all documents to the Personnel Department – NO LATER THAN 5:00PM, FRIDAY, OCTOBER 14, 2022. Any documents received after this date will not be considered for the Open Enrollment period.
Documents may be submitted via fax at 530-934-6452, email to gcpersonnel@countyofglenn.net, mail or in person at 525 W. Sycamore Street, Willows, CA 95988.
Please note – if you are currently receiving the Cash In-Lieu Benefit and wish to continue this benefit – you must re-certify your proof of other health coverage and fill out a new cash in-lieu enrollment form (attached). Failure to submit proof of other health coverage along with the enrollment form by Friday October 14, 2022 will result in loss of benefit.
ANCILLARY BENEFITS - Enrollment options for Dental, Vision and Life will be available during the ancillary benefit Open Enrollment period in November. An email with the details will be sent in October as a reminder.
