The Housing Services Office of Behavioral Health Care Services is pleased to announce the opening of a new affordable housing opportunity in Fremont, CA. Cottonwood Place is a brand new affordable senior development for persons 62 years and older containing 89 one bedroom units and 8 two bedrooms. The one bedroom average square footage is 612 and the two bedroom average square footage is 927. It is also a 100% non smoking building. 10 units are designated for MHSA-eligible seniors and 24 units are designed for frail seniors. Eden Housing Management, Inc. will manage the property. In addition, a full time Resident Manager will live at the building and act as primary management contact for the residents.

Frail units are designated units within the property designed for occupancy by households with one or more members who is frail. A person with any of the following would be considered frail: 1) Need for assistance with three or more Activities of Daily Living (ADLs) – self-care functions and housing management activities necessary for coping with life in a satisfactory manner or 2) Dementia or complex medical conditions like congestive heart failure, chronic lung disease, diabetes with complications, chronic kidney disease or renal failure. To qualify for frail units, individuals must note on their application that they believe they qualify for these units and then the applicant must provide a health care provider’s opinion that the applicant meets at least one of the two frail criteria.

There are a number of residential and community services that will be provided at the development:

On Lok Lifeways is a comprehensive health care plan for frail seniors. On Lok Lifeways provides care for eligible seniors, who wish to remain living at home in the community, with a complete program of health and health-related services. Our program includes preventative, primary and acute medical services and long-term care. Our primary focus is on preventative measures to maintain the health and well-being of our participants. Our goal is to help these seniors live independently, in their homes for as long as possible.


Mental Health Services Act or "MHSA" units are designated units within the property set aside for occupancy by households with one or more members with a serious mental health issue and a history of being homeless or at imminent risk of homelessness. To qualify for MHSA units, individuals must note on their application that they believe they qualify for these units and then the applicant must obtain certification of eligibility from the Alameda County Behavioral Health Care Services Housing Services Office (HSO). Certification requires the completion of a form and the submission of documents verifying the applicant has a serious mental health issue and is currently homeless, at-risk of homelessness, or being referred from specific programs serving this population.
