Reminder: COVID-19 Mobile Vaccination Survey (Please Respond by 7/27)

Dear COVID-19 Vaccine Provider,

Are you offering or planning to continue offering COVID-19 vaccinations?

Our Provider Survey closes in 1 week, on Tuesday 7/27.  We have already heard from over 100 providers like you! If you already completed the survey, there is no need to fill it out again as your response has been recorded.

If you have not yet responded, please take 5-8 minutes to complete this survey as it pertains to your services in Maricopa County, Arizona, especially if you plan to offer mobile COVID-19 vaccination services.

If you are the point of contact for multiple onboarded facilities, please fill out a form for each location that is offering mobile vaccination services. If none of your locations are providing mobile vaccination services, a single form is sufficient.
