Information collected in this survey will be kept confidential.
Demographic questions such as your ZIP code, age and race/ethnicity help us identify how issues may affect some groups of people more than others. No personal identifying information will be collected, and your answers cannot be traced back to you.

Peoria High School for ages 12+
When: June 15th, 17th and 19th (please check scheduler for times)
Where: Peoria High School Practice Gym, 11200 N 83rd Avenue, Peoria 85345
Vaccine: Pfizer (two dose)
Populations served: Vaccine is being offered to all 12+ population in accordance with CDC guidelines
Details: Hosted by City of Peoria & Albertsons/Safeway Pharmacy
Appointments are preferred, but walk-ins are welcomed. Please come prepared with a face mask, official photo ID, insurance card if you have one. There are no out-of-pocket charges for vaccine.
Registration:

- Printing out consent form and filling out prior to event will reduce registration time at event.
- Minors age 12-17 years old must have a parent or legal guardian sign the consent form on their behalf and have a parent, legal guardian or non-custodial adult parent and witness the vaccination.
- Non-custodial can be defined as a relative or any adult present with the minor.
