Vaccine: Janssen (Johnson & Johnson One-Dose)

Populations served: Open to all 18+ population

Details: Hosted by City of Peoria & Albertsons/Safeway Pharmacy

Please come prepared with a face mask, official photo ID, and insurance card if you have one

Where: Chase Field Downtown Phoenix (The clinic will take place indoors at the Third Base Suite Lounge – Enter through Chase Field parking garage)

Vaccine: Moderna

Populations served: All Adults over age 18

Details: Hosted by Fry’s/ Kroger

The vaccine offered at this clinic is Moderna for those 18 years and older. The second dose appointment will be scheduled at the time of the 1^st dose appointment booking. Please bring ID and insurance card information (medical and prescription insurance) with you to your appointment.

Details: Hosted by Albertson’s/Safeway

This event is by appointment only. You must be 16 years of age or older to receive the Pfizer vaccine and 18 years of age or older to receive the Janssen (J&J) vaccine. Please come prepared with a face mask, official photo ID, and insurance card. If you are 16 or 17, you must be accompanied by a parent or legal guardian.
