City of Phoenix - Harmon Park

Where: Harmon Park Address: 1425 S 5th Ave Buckeye Rd, Phoenix, AZ 85003 When:   Wednesday 3/17/21 - Tuesday 3/23/21 Vaccine:   Moderna Populations served: Residents of Phoenix who are age 55+  Details: Hosted by City of Phoenix, U of A and Maricopa County  This event is by appointment only. You must be 55 years of age or older and a resident of Phoenix in order to receive the vaccine. Please come prepared with a face mask, official photo ID, insurance card if you have one, and proof of appointment (appointment code). This event is only for the first 1st dose of Moderna. Some recent or upcoming events include:  Phoenix Union School District, Gila Bend, Town of Guadalupe, Chandler Unified, Glendale Community College, Mayo Clinic, Mesa Community Center, Rainbow Valley, Tolleson, Moon Valley High School, Independence High School, and many more.

MARICOPA COUNTY ELIGIBILITY: As of 3/2/21, Maricopa County is vaccinating the following eligible populations: Healthcare and EMS workers Long-term care facility residents & staff Adults 55 and Older  K-12 Teachers and Staff Childcare workers  Law Enforcement/Protective Services Workers
