This event is by appointment only. You must be 55 years of age or older and a resident of Phoenix in order to receive the vaccine. Please come prepared with a face mask, official photo ID, insurance card if you have one, and proof of appointment (appointment code). This event is only for the first 1st dose of Moderna. If you cannot provide proof of eligibility (age 55+) you will be turned away.
