COVID-19 Vaccination Survey

The State of Alaska wants to make sure that everyone living in Alaska can access the COVID-19 vaccine. Help us understand how to make the COVID-19 vaccine easily available to people with disabilities.
We want to hear from
- Adults with disabilities
- Family members/caregivers of individuals with disabilities
- Disability service providers
The online survey will take 10-15 minutes to complete. The survey is completely voluntary. Survey responses are anonymous. Please complete this survey by September 6, 2021.

Purpose of Survey
Survey responses will inform strategies to make the COVID-19 vaccine more easily available to people with disabilities. Responses will be used to advise messages and information about the COVID-19 vaccine in ways that are helpful and accessible.
