SDS E-Alert: Update on Use of Appendix K Covid-19 Person-Centered Support Plan Request (“Short Form”)
The Division of Senior and Disabilities Services (SDS) implemented the Appendix K Covid-19 Person-Centered Support Plan Request (also known as the “Short Form”) in March 2020 in response to the need for flexibilities during the COVID-19 pandemic. The Appendix K Covid-19 Person-Centered Support Plan Request allows recipients, guardians, and Care Coordinators to complete a shorter renewal support plan form when they are requesting the same services and same level of services that were approved by SDS the prior year.
SDS will continue to allow for the submission of the Short Form until October 1, 2021. This means that renewal support plans with start dates prior to or on September 30, 2021, that have no requested changes from the prior year’s approved services can be submitted on the Short Form. This includes renewal plans in which the previous renewal was also on the Short Form.
If services in a support plan need to change (type, amount, provider), a regular support plan (Uni-02) or a support plan amendment (Uni-03) needs to be submitted instead. Please be aware that SDS may request to see documentation of updated goals and objectives. Regardless of the type of form used, all renewal support plans must be submitted within regulatory timelines.
