Anyone can file a complaint against a licensed physical therapist or physical therapist assistant through the Department of Health (DOH). Complaints may be submitted anonymously. So, what happens next? And what are your options once you are notified?
A complaint can be submitted through the DOH website by visiting the File a Complaint page. The complainant selects the "Health Care Provider" option and provides details such as the licensee’s name (if known), relevant dates, and a description of the concern.
Once the DOH receives a complaint, department staff create a report for the Board to review. The department determines whether the Board has jurisdiction and whether the complaint contains allegations that, if true, would violate Washington law. If the complaint does not meet these criteria, the case is closed. If it does, an investigation begins.
If an investigation is opened, you will receive formal notification and a summary of the complaint. You will be asked to respond in writing within a set timeframe. You have the right to retain an attorney to assist with your response.
While the response is pending, the Board, DOH staff—including investigators and attorneys—and the Office of the Attorney General review the evidence to determine whether a violation occurred. If no violation is found, the Board will close the case. If a violation is identified, the matter is returned to the Board for review and approval of further action.
Possible disciplinary actions include fines, additional training, counseling, practice restrictions, or license suspension. If you disagree with the proposed action, you may request a hearing before a health law judge to appeal the decision.
Each complaint is evaluated on its own merits. The Board has a duty to protect the public and treats every complaint seriously. The time required to resolve a case varies, depending on the complexity of the issues and whether legal proceedings are involved.
