1* – Order ID (new)

2* – Sample ID(s) – moved to order choice level.  May have multiple sample IDs on single order/order choice.

3 – Received date – moved to order choice level

4 – Approval – date and initials of when/who order choice approved

5 – Completion date – date all order choices on order were finalized

*These IDs can be used to look up orders and samples in the system (so can Submitter ID(MRN) and Submitter SID as well as name and DOB)
