If you are using an alternate EVV vendor, you can check your Provider Medicaid ID in the Sandata Aggregator by following these steps:

1. Sign in to https://evv.sandata.com.
2. Click Reports.

3. Select a Corporation, Report Type, and Report Name.
   1. Corporation = should only have one selection available.
   2. Report Type = Daily Reports.
   3. Report Name = Provider Listing.

4. Select your account, then click Run Report.

5. The Provider Medicaid ID number will appear on the far left-hand side of the report.
