Dear New Yorker:

Our records indicate that you received unemployment or pandemic benefits in 2021. This email is to inform you that your 1099-G tax form is now available to download. If you have already downloaded or requested your 1099-G tax form, you can disregard this email.

You must include the information from your 1099-G tax form with your tax filing. You can download and print your 1099-G tax form directly from our website:      

1. Log in to labor.ny.gov/signin with your NY.gov username and password
2. Click ‘Unemployment Services’
3. Select ‘2021’ from the dropdown menu and click ‘View/Print your 1099-G’

If you do not have an online account, you can call 1-888-209-8124 to request your 1099-G form through our automated phone service. You will not need to speak to an agent – instead, follow the prompts to have your form mailed to your address on file.

As a reminder, unemployment benefits are considered taxable income. If you are still collecting unemployment benefits, we encourage you to review your records and, if necessary, adjust your tax withholdings. For more information, visit on.ny.gov/1099-G.
