1. PRE-REGISTER FOR ONE DAY ONLY COVID19 TESTING ON TUESDAY MAY 19 BY PICKING UP A PACKET AT THE FRONT DOOR OF CITY HALL; TESTING WILL BE THE FOLLOWING WEEK ON TUESDAY MAY 26 2020

  2. PREREGISTRATION IS REQUIRED. **QUANTITIES ARE LIMITED** PACKETS WILL ONLY BE AVAILABLE TO THE FIRST 150 RESIDENTS. IT WILL BE FIRST COME, FIRST SERVE

  3. THIS TEST IS OPEN TO ALL BAKER RESIDENTS, THERE ARE NO AGE RESTRICTIONS

  4. DRIVERS LICENSE & INSURANCE CARD WILL BE REQUIRED BOTH THE DAY OF TESTING AND A COPY MUST BE INCLUDED IN YOUR PACKET WHEN YOU RETURN IT

  5. BEGINNING ON TUESDAY MAY 19 AT 9 A.M. PACKETS WILL BE AVAILABLE FOR PICK-UP AT CITY HALL AT THE FRONT DOOR OUTSIDE FOR YOU TO TAKE HOME & COMPLETE

  6. YOU MUST RETURN YOUR PACKET NO LATER THAN 12:00 P.M. THURSDAY MAY 21, 2020 ALONG WITH A COPY OF YOUR DRIVERS LICENSE & HEALTH INSURANCE CARD IN ORDER TO PRE-REGISTER. 

    1. YOU CAN SCAN AND EMAIL THESE PACKETS TO AWILLIAMS@CITYOFBAKERLA.COM 
    2. YOU CAN DROP OFF YOUR COPIES AT THE FRONT DOOR WHERE YOU PICKED YOUR PACKET UP
    3. YOU MUST INCLUDE A COPY OF YOUR DRIVERS LICENSE AND INSURANCE CARD
  7. TESTING WILL BE CONDUCTED THE FOLLOWING WEEK ON TUESDAY MAY 26 AT ADVANTAGE CHARTER SCHOOL LOCATED AT 14740 PLANK ROAD, BAKER LA

  8. THERE WILL BE NO ON SITE REGISTRATION. 

  9. IF YOU DO NOT PRE-REGISTER YOU WILL NOT BE TESTED.

Please note that the City of Baker is only facilitating access to testing; Southeast Community Health Systems will be conducting testing.

***NO INFORMATION RELATIVE TO PATIENTS, TEST RESULTS OR APPOINTMENTS WILL BE MANAGED OR DISCLOSED TO THE CITY OF BAKER.***
