Options for Getting Samples to the State Laboratory (Monday through Friday)

1. Use any commercial carrier (Postal Service priority mail, UPS, Federal Express, etc.)

2. Use Laboratory Courier Service (no charge); see our website for more information http://dphhs.mt.gov/publichealth/LaboratoryServices/CourierRoutesSampleDelivery

3. Hand-deliver samples to the laboratory. Deliver to the new Laboratory Loading Dock located behind the Cogswell Building (1400 E. Lockey Ave). Turn south at the corner of Lockey and Roberts Street. Drive a short distance to the end and make a left into the parking area behind the building. Loading dock is straight ahead. See the map below and go to the star:

• The star on the map is the “new” loading dock location. Go to the brown double doors on the side of the new loading dock. There is a sign on the door that provides a telephone number. Please call that number and a person will come out and take your sample. In the near future, there will be a buzzer installed by the double doors (like the old entrance). At that time, you will just push the buzzer for assistance.
• You can also use the 1400 Broadway entrance, which has two 15-minute parking spaces by the Cogswell Building. It is badge access only, so please call 444-2642 when you reach the door, and someone will come to pick up your samples.

Online Payments

• Online payments now require an account to be created before payment can be made. If you do not want to create an account to pay your invoice online please reach out to our billing office at 406-444-2642 to pay your invoice. Credit/Debit card information will be required over the phone as well as phone number, billing address and an email address to send the receipt to. Remember a small fee is charged for paying with a credit/debit card. If you decide not to pay invoices with a credit/debit card we are still accepting payment by check and that can be mailed to:
