May 31, 2022 Renewal Announcement
The renewal notice packet for speech-language pathologist and audiologist licenses with a 05/31/2022 expiration is scheduled to be mailed out on 03/28/2022. If you changed your address since your last renewal and did not notify MDH, you may not get your renewal packet. Please follow the address change instructions if you need to update your contact information with MDH.
If you do not receive your renewal notice packet by 04/08/2022, please send an email to Health.SLPA@state.mn.us with the subject “Request for Renewal Notice.”
