Medicaid coverage protections are ending, and you need to provide information in an effort to keep your health coverage. The Indiana Family & Social Services Administration’s Division of Family Resources recently sent you a redetermination packet.
FSSA needs you to complete the information and return the packet to determine if you still qualify for coverage. Please return the packet by the due date listed.
Packets can be completed online in the Benefits Portal at https://fssabenefits.in.gov/, faxed to 800-403-0864, mailed to FSSA Document Center, PO Box 1810, Marion, IN 46952, or dropped off at your local Division of Family Resources Office, which you can find here: https://www.in.gov/fssa/dfr/ebt-hoosier-works-card/find-my-local-dfr-office/
