NR 812 Revisions – Revised Forms Available, and New and Revised Draft Factsheets Ready for Public Input

To all licensed drillers and pump installers,

Changes to NR 812 – Wisconsin’s well and pump code – have arrived!

Revisions to chapter NR 812, Wisconsin Administrative Code (NR 812) went into effect on July 1, 2020. DNR is sending weekly email communications on specific topics to all drillers and pump installers. This week, we focus on revised forms, and new and revised factsheets that are currently available for public input.

Key changes to forms and factsheets:

Forms and existing factsheets have been revised to make them consistent with the revisions to NR 812.  In some cases, only the code reference has changed.  In many cases, the changes to separation distance requirements were also revised. Two new factsheets will also soon be available.

The revised forms are now available at the following links: 

  1. Form 3300-076 - County / State Well Location Permit Application
  2. Form 3300-077First Water Quality Test
  3. Form 3300-265 - Pump Work – Water Test Request
  4. Form 3300-316 - Spring Box Application
  5. Form 3300-317 - Potable Nonpressure Storage Vessel Application

In addition, the online Well Construction Report and Well Filling and Sealing Report have been updated to be more user friendly and provide greater accuracy while incorporating needed changes to accommodate the revision to NR 812. A WAMS ID is required to access these two online forms.

  3. Draft - Flowing Wells (DG-099 – New)
