2026 Hospital Annual Reports

As part of the Division’s commitment to Teach, Learn and Collaborate, the Division of Quality Assurance (DQA) is providing notice to hospital providers regarding the current annual reporting period. Hospital licensees are required to submit their annual report using the DQA form and pay the fee as required under Wis. Stat. § 50.355 and Wis. Stat. § 50.135(2)(a).

On August 13, 2026, DQA sent out 2026 hospital annual reports and fee invoices electronically to the contact email on file for each hospital. These annual reports and fees are due no later than October 15, 2026. Hospital licensees are required to submit their annual report using the DQA form and pay the fee as required. If you have received that email, and not yet replied to confirm receipt, please do so, or a paper copy will also be mailed to your hospital’s mailing address.

Failure to submit the required report and fee by October 15 will result in a $10 per day late fee based on the postmark date of the materials. Further failure to submit these materials may result in revocation of your hospital license.

To ensure accurate accounting and avoid late fees and possible collections actions, please follow these steps:

1. Make checks payable to the Division of Quality Assurance.
2. Print the invoice and send a copy with your check.
3. Ensure your license number is printed on the check and/or check stub.
4. Send all payments to:
