VFC Annual Re-enrollment

This message is being sent to VFC providers, local health officers, Tribal health directors, and select DHS staff. 

Annual re-enrollment for all VFC providers will begin on Monday May 4, 2026, and will go through the close of business on Friday, May 29, 2026. As in the past, all VFC sites must re-enroll via the Wisconsin Immunization Registry (WIR). If re-enrollment is not completed by the May 29, 2026, deadline, your site may be suspended from ordering vaccines through the VFC program.

Every site that receives and administers VFC vaccines is required to complete and submit VFC re-enrollment using WIR. The Acknowledgment Page from the VFC Acceptance Agreement must be returned to the Wisconsin VFC Program via email vfc@wi.gov or fax 608-267-9493 by the May 29 deadline.

Instructions for Re-enrollment

  • Open WIR and log in. Users with IR Administrator, IR Inventory Control, or IR Typical User roles may complete VFC re-enrollment.
  • If you have access to more than one clinic in WIR, click the link of the clinic you plan to re-enroll first. Each VFC clinic will need to be re-enrolled separately in WIR.
  • Once logged in, click on the tab titled "r/r" on the top left of the menu bar.
  • Scroll down to the section titled VFC Renewal and Registration Forms
  • To view a copy of the instructions from the WIR User Manual, click on the link "VFC Registration and Renewal Instructions.”
  • To start your re-enrollment, click on the link "Begin Online VFC Registration or Renewal."
  • On the next screen, choose the site you want to re-enroll from the dropdown menu and click Select Site.
  • On the following screens, all fields appearing in blue are required to be filled in. Some information may be pre-filled from data already in WIR. Please check all fields for accuracy.

Note: As in previous years, WIR will time out if left idle for 60 minutes. As long as you are actively working on your re-enrollment and click continue within the 60 minute window, it should not time out. However, if you do time out, you will encounter a red banner error screen notifying you to contact the WIR helpdesk for further assistance. It is best to make sure you have the time to complete re-enrollment once you start the process.

Save, Submit, and Acknowledge

  • After completing the above steps, click "SAVE AND SUBMIT."
  • All providers are required to sign and return the final “Acknowledgement Page” before the re-enrollment will be considered complete.
  • To access the full VFC Acceptance Agreement, click on the blue VFC Acceptance Agreement link on the Application Submitted–Almost Done page. This will be your only chance to print and save this document. (See screen shot below).
  • If you choose not to download and print the agreement at this time, you will have to re-do your entire re-enrollment in WIR.

Once the agreement is printed

  • Read through both the Provider Agreement and the Restitution Policy. The last page of the agreement is the Acknowledgement Page.
  • You are required to check both of the acknowledgement boxes, sign, and return the last page only. Do not return the entire agreement. 
  • Keep the acceptance agreement packet with your VFC records.

By checking the acknowledgment boxes and signing the agreement, you agree to adhere to the requirements outlined in both the Restitution Policy as well as the VFC Provider agreement. If you do not check both boxes and sign the Acknowledgement Page, your VFC re-enrollment will not be approved, which will put your site at risk of being suspended from ordering vaccine.

The signature page must be emailed to the Wisconsin VFC Program at vfc@wi.gov or fax 608-267-9493 by the May 29, 2026, deadline.
