Transition to New ePOC System

As part of our commitment to Teach, Learn and Collaborate, the Division of Quality Assurance (DQA) is sharing information on the upcoming transition to the new electronic Plan of Correction (ePOC) process for nursing home providers.

In July 2025, the Centers for Medicare and Medicaid Services (CMS) transitioned to the Internet Quality Improvement and Evaluation System (iQIES) for completion and documentation of the long-term care survey process. iQIES includes access for providers to receive a Statement of Deficiency (SOD), write and transmit a POC, and share supporting documentation directly through the system. The new ePOC process will streamline communications and enhance the security of confidential information.

Wisconsin will begin using this process in the coming months. All nursing home providers must follow the iQIES Access Instructions for Long Term Care Providers Using ePOC to ensure that the person(s) responsible for receiving SODs and writing POCs has the appropriate login credentials and user role identified.
