Update – Continued availability of PDF BID Form

As part of the Division’s commitment to Teach, Learn and Collaborate, the Division of Quality Assurance (DQA) is pleased to provide additional information about the E-Background Information Disclosure (eBID) webform, F-82064.

A downloadable, fillable PDF version of the form will remain available and will no longer be phased-out. The PDF form has been updated to align with the eBID webform and to comply with regulatory and digital accessibility standards. Providers are encouraged to use the eBID webform but may also choose to use the downloadable BID PDF form.
