Harnessing innovation and strengthening partnerships to build a healthier future for rural Wisconsin
DHS seeks applicants for Rural Health Transformation Program Advisory Council
The Wisconsin Department of Health Services (DHS) is seeking qualified applicants to serve on the Rural Health Transformation Program Advisory Council. The council will provide strategic guidance and input to DHS as a part of the state’s implementation of the program. Applications are due Friday, February 27, 2026.
DHS was awarded $203.7 million from the Centers for Medicare and Medicaid Services (CMS) for the first year of the program. The program is a federally funded initiative to strengthen rural health care systems and improve access, quality, workforce capacity, and care coordination across Wisconsin’s rural communities. Partner input was critical to shaping Wisconsin’s program application and will be essential to successful implementation.
Throughout February, DHS will accept applications for the advisory council, which will include 17-20 members representing the breadth and geographic diversity of partners engaged in rural health initiatives. This council offers an opportunity to contribute to shaping how rural health priorities are addressed, bringing diverse provider, community, and lived experience perspectives to the table.
DHS is specifically seeking applicants who represent either of the following:
Provides or supports the delivery of health services in rural Wisconsin. This includes:
- Those who provide services in rural hospitals, rural health clinics, community health centers, long-term care facilities, at local or Tribal health departments, or through county human services.
- Individual providers who practice in rural communities, including but not limited to primary and behavioral health providers, physicians, nurses, pharmacists, EMS, community health workers, and peer support specialists.
- Organizations that support health care providers.
Have lived experience with rural health care.
- This includes people who receive care in rural Wisconsin at a rural hospital, rural health clinic, community health center, long-term care facility, or through a local or Tribal health department or county human services agency.
Appointments to the council will be made by the DHS Secretary, with an emphasis on ensuring geographic diversity and a range of perspectives from communities across the state. The council will exist for the length of the grant (2026-2031), and members will serve at the pleasure of the Secretary during that time.
Please fill out and submit the application by Friday, February 27, 2026, if you are interested in being part of the Rural Health Transformation Program Advisory Council.
