Effective Immediately: Address update for all mailing types for the Division of Quality Assurance - Central Office and Southern Regional Office
As part of the Division’s commitment to Teach, Learn and Collaborate, the Division of Quality Assurance (DQA) would like to make you aware of the upcoming change in address for all delivery types, regarding the move of our Central Office/Southern Regional Office (CO/SRO), located in Madison.
On August 25, 2025, DQA CO/SRO, relocated to 201 E. Washington Avenue, Madison, Wisconsin, 53703.
Effective immediately, for all mailing types, including USPS, and certified mail and packages, which are delivered via Fed Ex, UPS, and Spee-Dee, you must use the following address:
