CLTS Program Manual Update for FMS Providers

The Wisconsin Department of Health Services (DHS) has made changes to the Children's Long-Term Support (CLTS) Program Manual, P-02256, (PDF) that impact providers that offer financial management services (FMS).

Clearly defining “authorized representative”

The term “authorized representative” has been changed to “legal representative.” A child’s legal representative is defined as an individual with court-ordered authority to make major medical decisions and/or consent to CLTS Program enrollment and services on behalf of the child. If there is no parental involvement, only the legal representative may sign CLTS Program documents.

FMS clarifications

Under basic level:

  • FMS providers are required to train caregivers on service requirements, limitations, and exclusions for the CLTS Program service(s) the caregiver will directly provide.
  • FMS providers must complete caregiver screening activities when the caregiver is hired and when the FMS Provider renews their Medicaid Provider Agreement.

Under enhanced level:

  • FMS providers must verify a caregiver’s qualifications and maintain documentation that the caregiver has received required participant-specific training.
  • CLTS Program funds may not pay for services provided by a person who has had their professional license or certification denied, suspended, or revoked.

This update is part of DHS’ effort to keep the CLTS Program Manual current and consistent with state policies. Policy changes will be effective on October 1, 2025. For questions about these policy changes, please email CLTS Provider Relations at DHSCLTSProviderRelations@dhs.wisconsin.gov.
