CLTS Resources Webpage Update on the ForwardHealth Portal
The Wisconsin Department of Health Services (DHS) has updated the Children's Long-Term Support Program (CLTS) County Waiver Agency and Provider Resources webpage found on the ForwardHealth Portal. County waiver agencies (CWA) and CLTS providers can find support resources for CLTS prior authorizations and claims using the ForwardHealth Portal.
What’s new?
A new web address (URL) has been created for the resource webpage. This is no longer considered a transition webpage and will continue to be updated with new resources for the CLTS Program.
CWAs and CLTS providers are strongly encouraged to bookmark the new URL: https://www.forwardhealth.wi.gov/WIPortal/cms/page/message/clt-tp-resources
