2025 Nursing Home Annual Reporting

As part of the Division’s commitment to Teach, Learn and Collaborate, the Division of Quality Assurance (DQA) is providing advanced notice to nursing home providers of the upcoming annual reporting period.

Wis. Stat. § 50.03(4)(c)2 of the Wisconsin State Statutes states that every 12 months, a nursing home licensee shall submit a report in the form and containing the information that the department requires, including payment of the fee required under Wis. Stat. § 50.135(2)(a).

On or before August 1, 2025, all nursing homes will receive their annual report packet in the mail. Nursing homes must review the information contained in the reports and make note of any changes on the forms provided. These forms must be returned no later than October 1, 2025, to dhsdqabnhrclicensing@dhs.wisconsin.gov. Failure to submit the required information may result in revocation of your license.

In addition to the annual report, providers must pay an annual inpatient health care facility fee. The amount due to the department will be included with the annual report inpatient health care facility fee notice. Your packet will contain an addressed envelope to mail your check and the remittance slip. Payment is due by October 1, 2025. Payments received after this date, will be assessed a $10.00 per day late fee.

To assure accurate accounting, and to avoid late fees and possible collections actions, each provider shall:

  1. Make checks payable to the DIVISION OF QUALITY ASSURANCE
  2. Send the fee remittance slip with your check
  3. Ensure your license number is printed on the check and/or check stub
  4. Mail your payment using the enclosed envelope
