Fact Sheet: Multiple Dates of Service on a Claim

The Children's Long-Term Support (CLTS) Program has created a fact sheet to help providers add multiple dates of service to a claim. Providers can submit claims with multiple dates of service, also known as date span billing, if the prior authorization and claim meet the proper criteria.

This fact sheet includes:

• Criteria for date span billing
• How to bill consecutive days on a claim
• Same service billing for nonconsecutive days
• Billing for overnight services
• Billing for services with “monthly" or “session” units
