New and Updated Documents Available for the ForwardHealth Portal Transition

The Wisconsin Department of Health Services (DHS) has made updates to current resources and developed new resources to support Children's Long-Term Support (CLTS) Program providers and county waiver agencies in the transition to using the ForwardHealth Portal for prior authorizations (PA) and claims.

These updates can be found on the ForwardHealth Children's Long-Term Support Program: Claims Processing Transition webpage.

Updated Resources

CLTS Place of Service (POS) Codes Table: Now includes a reminder to use the GT modifier when submitting claims for remote (POS 02) services.
FAQs About CLTS Program: Third-Party Administrator Transition: New questions have been added.

New Resources

Payments for CLTS Program Services: Information about the two payment options for CLTS Program services.
