ForwardHealth PA Diagnosis Code Reminder

Diagnosis codes are required on prior authorizations (PAs). County waiver agencies may use a generic International Classification of Diseases (ICD-10) diagnosis code of Z418 on PAs or submit a diagnosis code specific to the participant.

All diagnosis codes must be an ICD-10 code and must be submitted without the decimal. Diagnosis codes with a decimal point submitted through the PA flat file upload or the direct entry PA option will not be accepted.

ForwardHealth has auto-assigned a generic ICD-10 diagnosis code of Z418 to all PA history transferred from WPS.
