CLTS Prior Authorization Conversion for the TPA Transition for ForwardHealth Portal

The Wisconsin Department of Health Services (DHS) has transitioned from Wisconsin Physicians Service (WPS) to ForwardHealth, the state’s fiscal agent, for Children's Long-Term Support (CLTS) Program prior authorizations (PAs) and claims processing. PAs and claims should now be submitted through the ForwardHealth Portal.


Matching provider tax IDs and Medicaid IDs

One year of PA history has been sent to ForwardHealth for conversion and is available in the ForwardHealth Portal for CLTS providers to claim against. Many of the PAs have already been successfully converted. DHS needs your help to convert the rest of the PAs.

The PAs that have not been converted include CLTS providers with more than one servicing location, and, therefore, more than one Medicaid ID. DHS needs the county waiver agencies' (CWAs’) assistance in identifying the correct Medicaid ID for those PAs.


Action required

CWAs received an email from the CLTS Operations Team with an Excel file containing PAs that have not been converted specific to their CWA.

The email contains detailed instructions on how to complete the Excel file of PAs. Once complete, CWAs should send the file back to the CLTS Operations Team.

Completing this task as soon as possible, and no later than May 30, will increase the number of PAs that be can converted into the ForwardHealth Portal and made available for CLTS providers to submit claims against.
