ForwardHealth Prior Authorization Conversion Process
The Wisconsin Department of Health Services (DHS) has transitioned to using the ForwardHealth Portal for the Children's Long-Term Support (CLTS) Program prior authorizations (PAs) and claims. DHS is sharing important updates on PAs.
PA conversion continues
One year of PA history, from April 2024 to April 2025, is being converted from Wisconsin Physicians Service (WPS) for CLTS providers to view and submit claims to in the portal. The conversion process assigns the CLTS provider’s Medicaid ID and a Gainwell-assigned PA number.
The PA conversion is not complete. As a result, CLTS providers may not see all their PAs from WPS yet. PAs continue to be converted daily and DHS anticipates most WPS converted PAs will be available by May 16.

Not finding PAs after May 16?
If you are expecting converted PAs and are not finding them after May 16, contact the CLTS Operations Team at cltsoperations@gainwelltechnologies.com or 844-942-5870 for assistance. The CLTS Operations Teams will ask questions to help them locate your PAs. If PAs cannot be located, CLTS providers will be encouraged to work with their county waiver agencies to have new PAs created.
