Questions About the CLTS Program or Providers? Make Sure They Get to the Right Place
The Wisconsin Department of Health Services (DHS) is transitioning from Wisconsin Physicians Service (WPS) to Gainwell Technologies, the state’s fiscal agent, for Children's Long-Term Support (CLTS) Program prior authorizations (PAs) and claims processing using the ForwardHealth Portal.
This transition has caused confusion about who to contact and where to send questions. To get your questions to the right place, follow these guidelines.
The CLTS Provider email (DHSCLTSProvider@dhs.wisconsin.gov) is for:
- Questions about using the CLTS Program Provider Registry.
- Questions about provider registration status.
- Provider communication about the registry and directory.
The CLTS Provider Relations email (dhscltsproviderrelations@dhs.wisconsin.gov) is for:
- CLTS providers requesting information on CLTS program policy, operations, or CLTS waiver services.
- Questions about provider PA issues.
- Provider integrity concerns.
- Timely filing waiver requests.
- Information to resolve provider payment issues.

The Gainwell CLTS Operations email (cltsoperations@gainwelltechnologies.com) is for help with:
- Logging into the ForwardHealth Portal.
