Investigating & Reporting Requirements
As part of the Division’s commitment to Teach, Learn and Collaborate, the WI Department of Health Services (DHS), Division of Quality Assurance (DQA), and the University of Wisconsin Oshkosh, Center for Community Development, Engagement and Training (UWO CCDET) are here to help you meet your training needs.
Developed by UWO CCDET in collaboration with DQA, these workshops are designed to support you in meeting the regulatory requirements for investigating and reporting misconduct in all regulated provider settings. To ensure affordability and accessibility, the workshops are partially subsidized by DQA. See below for more information and register today!

For Assisted Living, Homecare, Mental Health, and Substance Use Treatment Providers:
Are you responsible for investigating and reporting allegations of misconduct in state-licensed/certified DQA-regulated facilities?
