Updated Instructions for “Transfer Level of Care Results to iC” Box within the CLTS Functional Screen
The instructions for transferring a Functionally Eligible screen to iChange within the Children’s Long-Term Support (CLTS) Functional Screen have been updated.
When a screener completes a screen that is calculated as Functionally Eligible for children’s programs, the “Transfer to iChange” box should be marked and the “Confirm” button should be clicked immediately per the guidance received in the February 2025 CLTS Functional Screen Teleconference.
When a screener completes a screen that is calculated as Not Functionally Eligible (NFE) for children’s programs, the “Transfer to iChange” box and the “Confirm” button will not appear. In instances of NFE, the following dialogue will appear: Email DHSCLTSFS@dhs.wisconsin.gov when the child/individual is Not Functionally Eligible (NFE) for all children's programs (except CCS and CRS).
