MyWisconsin ID and Pre-Registration for Access to the DQA Misconduct Reporting Systems

MyWisconsin ID is the State of Wisconsin's identity solution that allows citizens the ability to securely access participating online state services and systems using a single user ID and password. Beginning March 19, 2025, MyWisconsin ID will be needed to access the DQA Misconduct Reporting System. The existing WILMS logon will no longer work at that time.

To avoid interruptions in reporting access, entities/facilities should submit a pre-registration request before February 28, 2025 to authorize users for their entities/facilities. This process will require users to create a MyWisconsin ID and then complete a form identifying the entities/facilities for which they need to report. Access will no longer be limited to only two users per entity/facility. Pre-registered users will then be able to log into MRS using their MyWisconsin IDs beginning March 19, 2025.

Entities/facilities should continue to use their existing WILMS IDs or follow the existing WILMS registration process to access MRS until 5 p.m., on March 18, 2025.

Notice of Planned Outage
Please note that a planned outage for the Misconduct Reporting System will be scheduled from 5 p.m. on March 18, 2025, until 8 a.m. on March 19, 2025.
